Provider Network Information

  • If you are seeing providers in Kitsap, Mason, Jefferson and Clallam counties, you must use the KPS Health Plans Network. For providers outside the KPS Health Plans Network, please refer to our extended network websites: 

  • First Choice Health Network (FCHN): all of Washington, Idaho, Montana, Alaska

  • Providence Network: Southwest Washington (Clark, Cowlitz, Klickitat, Skamania & Wahkiakum Counties and parts of Oregon) Choose Providence Preferred Providers (PPO) in Box 1.

  • MultiPlan Network: all of the U.S. outside Washington State.

Provider Nomination

  • The KPS Participating Provider network is constantly expanding to meet your needs. If you have a particular healthcare provider that you would like us to invite to join our network, we would like to hear from you. To nominate your provider, please complete and mail or fax the Provider Nomination Form to the KPS Provider Relations Department.

Note Regarding Provider Services

  • The inclusion of a specific provider on the KPS Preferred Provider List does not imply covered services under all KPS contracts. Please review your specific KPS Health Plans contract booklet for complete details about which benefits and services are covered.

  • Services are provided through your health care plan's delivery system. 
    The continued participation of any one physician, hospital or other provider cannot be guaranteed.
