	Summary of Benefits Large Group 50/50 Plan The Healthy Investor™ Large Group HSA Plan

• In order for KPS Health Plans to provide a group rate for large groups of 51+, we require the disclosure of a minimum of census information, as outlined in the Group Rate Request Form. KPS must receive census information in electronic format (Excel) to ensure fastest turnaround. If received in hard copy, add at least three additional days for the quoting process.
• Once you have selected a customized plan, you or your broker will be required to fill out a Large Group Master Application (customized with benefit information for your group) and Enrollment Application (PDF). One Enrollment Application must be completed and signed by each enrolling employee.
